post operative fever
introduction
there is normal variation in body temp up to 1c with max at eveining
any patient with post operative fever should be investigated as following
hx examin investig
the time at which pre operative or post operative fever may suggest it origin
- fever begin intra operative
a- preexisting sepsis with intraoperative septicemia
mg hyperthermia (rare genetic disorder high fever 41 c ms rigidity acidodsisif un treated l2 acidopsis
B-bl transfusion
C- heat stroke *rare*
IN THE IMEDIATE POST OPERATIVE PERIOD
a-atelectasis
b-thyroid crisi
c- adrenocortical insuffecincy
d-transfusion reaction
within 36hours:
it is unually2nry to pulmonay complic

3rd day fever
usually thrombophlebitits due to infection of an indwelling intravenous line
.
5-7day fever
1)thrombophlebitis in the lower limb
2)deep venous thrombosis
3)urinary tract infection
4)wound infection
5)intra-abdominal abscess
Diagnosis
1-full history
Age
*children :minimal infection:high fever
*elderly:significant infection :minimum fever
-Associated medical conditions
-type of operation
-time of fever
-Associated symptoms
2-full examination:Mouth ,throat,chest,abdomen ,intravenous lines ,lower limbs
3-investigations
-cultures
-cxr
-abdominal u\s
-urine analysis
