انا حبيت اضع في هذا الموضوع بعض المعلومات المهمة في الجراحة واللي ممكن تفوت على كتير مننا

Breast
1-In any breast mass or lesion u must exclude malignancy
2- History of trauma or acute inflammation dosent exclude malignancy
3-the only sure investigation is tissue biobsy
4- Some masses could be missed by physical exam or even by mammography
5- 75% of breast lymphatic drainage goes to axillary LN
6- Its possible to find 2ndries in axillary(T0N1) LN without primary lesion in the breast and these cases have better prognosis than other cases with 1ries
7-If u find axillary LN in female without breast lesion think in :
infections:ExAMPLE:IMN
drug allergy :as sulfa containing drugs or penicilline or iodine
lung cancer
lung cancer is very important to exclude as its of high mortality in females
8- more than tow thirds of breast cancer cases r postmenoposal
9- presistant headache or Back pain +cancer Breast suggests Metastasis so Do CT bone + CT Brain
10-Cancers by incidence in females :
breast then lung then colon
11- Cancers by Mortality in females :
Lung then breast then colon
12- the only effective 3 screening tests that should be done for femals:
A- Mammogram for breast
B-PAP smear for cervical carcinoma
C- colonic examination either by stool examination for blood or by physical exam

PartII-General Surgery-
After prolonged forcible Vomiting IF severe epigastric pain or heamoptysis ocuurs Suspect Perforation
Delay in Diagnosis or TTT has grave conseq. ……. Barium is Diagnostic and emergency surgical Repair is TTT
Early signs of bowel Obstruction : No gas or feces , abdominal distension , vomiting X-ray is D
But if strangulated Obstruction occurs :As above + fever , leucocytosis, signs of peritoneal irritation
In Atypical appendicitis CT is the Standerd
Cancer of Rt Colon is associated with Anemia ( no good reason ) + occult blood in stool
Cancer of Lt Colon is associated with Bloody bowel Movements + blood coats the stool
Chronic Ulcerative colitis (CUC) is Maneged 1st Medically
In all Anorectal diseases Cancer should be ruled out by proper physical Exam + Examine inguinal LN
Hemorroids------------- internal ------------------bleeds------------------ band Ligation
------------External ------------------ hurts (pain)------------- 1st medical if fails Surgery
Anal fissure ---------pain with defecation + steaks of blood on stool . pain is too severe to the extent that the patent avoids defecation .
Always remember The Only effective 3 screening Tests in female r:
--------PAP Smear ------------ Cancer Cervix
--------Mammography
--------stool examination For blood ------------Cancer colon
Advice: Always think in the rare presentation of the Common disease Rather Than Thinking In The Common presntation Of the Rare Disease.
